Is Rectal Ejaculation Real?
In 2021, doctors from the University of Texas Medical Center in Galveston, Texas published a case study about a case about a patient that passed urine with feces, feces with urine, and semen with feces (1). The Short Answer. This case study shows that while uncommon, it's possible for urine, feces, and even semen to come out of the wrong holes (1). A 33-year-old male presented with five days of testicular pain and a report of sperm passage from his rectum while ejaculating over the past two years. Imaging of the patient's pelvis revealed evidence of a rectal-prostate fistula. The patient was treated with piperacillin-tazobactam (an antibiotic) and a surgical fistula repair was performed. Further investigation uncovered that the man had spent nearly three weeks in a medically-induced coma due to a cocaine and phencyclidine overdose two years prior to this incident. Doctors think that a botched Foley-catheter placement may have been to blame - likely due to a premature balloon dilation in the man's prostatic urethra. Ouch! What is a rectal-prostate fistula? A fistula is an abnormal connection between two body parts, such as an organ or blood vessel and another structure. Fistulas are usually the result of an injury or surgery. Infection or inflammation can also cause a fistula to form. In this case, the patient in question had a fistula that formed between their rectum and their prostate, aptly called a rectal-prostate fistula. How common are rectal-prostatic fistulas? Rectal-prostatic fistulas have an incidence of about 0.5 cases per 100,000 people each year. In adults, these abnormal connections are usually caused by some sort of trauma, surgery, radiation, or an inflammatory condition like Crohn’s disease. They can also be caused by prostate cancer or prostate cancer therapy. Symptoms of RUF can be feces in the urine, urine in the feces, urinary tract infections and in this case, semen exiting the rectum. What caused this man to leak semen from the wrong hole? For this particular 33-year-old, the problem likely started when he was put into a medically induced coma after a drug overdose. It’s hypothesized that while he was in the coma, a Foley catheter was incorrectly inserted. A Foley catheter is a tube that’s inserted into a patient’s urethra to drain the bladder of urine. Catheters are extremely common for hospitalized patients, but as this case shows, anything that’s inserted into the body, must be done so with extreme care to avoid serious complications. How he knew something was wrong The patient finally went to the doctor because he was having intense pain in his testicles over the course of 5 days. It was at that point that he told the doctors that he’d been passing semen and urine from his anus for two years. He also said that he had experienced feces in his urine as well. After performing imaging on the man's pelvis, doctors found a fistula between the patient’s anus and prostate, which was responsible for his bodily fluids exiting from the wrong holes. How can I avoid a rectal-prostatic fistula? While this story probably makes you feel very uncomfortable, the good news is that doctors were able to patch the fistula through surgery. The patient was able to make a full physical recovery with the only side effect being reduced semen volume over several months. If he had not sought treatment, it’s likely that this condition would have become life-threatening. The authors of this case study concluded by saying, “This case not only highlights a rare complication of catheter use but also emphasizes the importance of provider mindfulness when utilizing seemingly benign therapies such as Foley catheters.” If you're experiencing health issues - go to a doctor! As this case illustrates, time is of the essense. Had this man waited any longer to seek treatment, it's entirely possible that this story would be even more grim. There are a number of common causes of testicular pain after sex (1): Epididymal hypertension (blue balls) Hydrocoele Inguinal hernia (scrotal hernia) Type I Prostatitis: Acute Bacterial Prostatitis Type II Prostatitis: Chronic Bacterial Prostatitis Type III Prostatitis: Chronic Pelvic Pain Syndrome Varicocele Epididymal Hypertension (Blue Balls) Epididymal hypertension, also known as blue balls, usually happens when a man who's aroused doesn’t have an orgasm. However, blue balls after sex or masturbation can also occur if an orgasm is withheld for a long period of time. This is true whether the orgasm is held intentionally through a practice known as edging and also if an orgasm is delayed unintentionally through prolonged sex or sexual teasing. The mechanism behind blue balls starts with increased blood flow to the penis and testicles during sexual arousal. Hydrocoeles are not usually painful, but they can cause discomfort if they are large. However, treatment is often recommended if the hydrocoele is causing discomfort or embarrassment. A hydrocoele can be treated by draining the fluid with a needle (aspiration) or by a minor surgical procedure called a hydrocelectomy. Inguinal Hernia (Scrotal Hernia) A scrotal inguinal hernia occurs when tissue, such as part of the intestine, protrudes into your scrotum through a weak spot in your abdominal or pelvic floor muscles. The resulting bulge can be painful, especially when you cough, bend over, or lift a heavy object. However, many hernias do not cause pain. Scrotal inguinal hernias can occur during sex due to increased intra-abdominal pressure combined with a weak spot in muscles. They can also occur when an existing inguinal hernia is made worse through sexual acts such as holding up a partner, thrusting your hips, grunting, or other common acts that occur during sex. An inguinal hernia isn't necessarily dangerous, but they do not go away on their own. Additionally, they can lead to life-threatening complications. Your doctor is likely to recommend surgery to fix an inguinal hernia that's painful or enlarging. Prostatitis after sex Acute bacterial prostatitis (ABP) from sex Acute bacterial prostatitis after sex happens when a urinary tract infection (UTI) causes bacterial growth in the prostate gland. Symptoms include fever and chills. You may also experience painful and frequent urination or have trouble urinating. Though acute bacterial prostatitis only accounts for approximately 5% of all prostatitis cases, it requires immediate medical treatment since it can lead to sepsis (3). Chronic bacterial prostatitis (CBP) from sex Chronic bacterial prostatitis can occur after sex due to recurrent urinary tract infections (UTIs). Similar to acute bacterial prostatitis, CBP occurs when bacteria become trapped in the prostate gland. Unlike ABP, CBP results in recurrent infections that are often difficult to get rid of. Chronic bacterial prostatitis also requires immediate medical attention as it can lead to other diseases or death in severe cases (4). Chronic pelvic pain syndrome, or CPPS from sex Chronic pelvic pain syndrome (CPPS), also known as non-bacterial chronic prostatitis, is the most common prostatitis type (5). It's similar to chronic bacterial prostatitis, but it does not come from a bacterial infection. Bacterial prostatitis and non-bacterial prostatitis (i.e. CPPS) are commonly misdiagnosed for each other, and both are poorly understood, difficult to treat, and cause a range of symptoms including pain, urinary problems, reduced quality of life and sexual dysfunction. Chronic pelvic pain syndrome from sex often happens because of stress from rough sex, nerve irritation, blunt force injuries, or damage from previous urinary tract infections. Varicocele (pronounced VAR-ih-koe-seel) A varicocele is an enlargement of the veins within your scrotum (the loose bag of skin that holds your testes). A varicocele is similar to a varicose vein you might see in your leg. Varicoceles are a common cause of low sperm production and decreased sperm quality, which can cause infertility in men. Varicoceles can also cause your testes to develop abnormally or even cause your balls to shrink. Since a varicocele usually causes no symptoms, it often requires no treatment. However, if you experience pain or swelling in your scrotum, discover a mass on your scrotum, notice that your balls are different sizes, develop a varicocele in your youth, or you're having problems with fertility, you should contact a healthcare professional. Testicular Bruising or Scrotal Trauma Perhaps the most common and straightforward cause of pain in your balls after sex is simply blunt force trauma to your scrotum or balls during sex. What Increases Sperm Count Before looking to scientifically backed supplements and medical treatments, which we will also cover, let’s take a look at some basic things men can do in their everyday lives to improve fertility and sperm count. Psychological stress can play a major factor in sexual health, including sperm count. “Many clinical studies looking at the effects of psychological stress on male fertility have shown that stress is associated with reduced paternity and abnormal semen parameters. Enough scientific evidence exists to suggest that psychological stress could severely affect spermatogenesis, mainly as a result of varying testosterone secretion. Foods That Increase Sperm Count Foods rich in antioxidants and Vitamins C and E have been shown to have positive effects on sperm motility, health, and general function. “Supplements such as CoQ10 and alpha-tocopherol significantly improve sperm count. Also, carnitine has positive effects on sperm motility and morphology. Simultaneous administration of vitamin E and vitamin C reduces the sperm DNA damage. However, in some studies, one or more factors have not changed substantially. In most of the studies, antioxidant supplementation improved the number, motility, morphology and sometimes DNA integrity of sperm. The present study showed that antioxidant supplements, especially a combination of antioxidants such as vitamin C, vitamin E, and CoQ10 intake can effectively improve semen parameters in infertile men.” (2) Some vitamin C rich foods include broccoli, oranges, and spinach. Foods containing high amounts of Omega-3 fatty acids can also improve sperm function (3). Foods like avocado, walnuts, and fish are great and healthy sources of Omega-3 fatty acids. How Long Does It Take To Get Pregnant After Sex A woman doesn’t immediatly get pregnant after sex. It can take up to six days after sex for the sperm and egg to join and form a fertilized egg. After fertilization it can take three to four days for the fertilized egg to completely implant itself in the lining of the uterus. Pregnancy begins during implantation, when the hormones needed to support pregnancy are released (4). Implantation cannot happen immediately after fertilization because it takes time for the fertilized egg to reach the uterus. One study suggests that the later a fertilized egg implants, the more likely early pregnancy loss becomes.The study found that implantation occurred on days 8–10 after ovulation for 84% of participants. Of the eggs that implanted after day 11, 82% resulted in pregnancy loss before 6 weeks (5). There are many things you can do to improve and maintain the health of your testicles, including diet, exercise, and even trying a testicle massage. They require TLC like any other organ and here’s how you can care for them. What is the optimal temperature for testicles Testicles are outside the body for a reason, to stay cool. Testicles function optimally at 35C or 95F. In fact, decreased testicular volume, decreased sperm count and decreased testosterone production were observed in male rat models with induced testicular temperature of 109F for just 45minutes (think hot tub exposure!). With this small amount of heat, testicular weight decreased by 65% and sperm count decreased 29% in the rat population (1). Are some foods better for testicular health than others Broad foods for testicular health include those rich in antioxidants, such as leafy greens, berries, broccoli, bell peppers, oranges, and eggplant. The antioxidants scavenge free radicals that can damage DNA of developing spermatids and impair production of fully functional sperm. Selenium along with Vitamins C and E also provide antioxidant services, to protect and promote sperm development, increasing sperm count and testosterone levels (2). Zinc is an even more important mineral to boost sperm count. Zinc is a natural local antimicrobial agent in the testicles and boosts testosterone production (2). Melatonin is an antioxidant as well, which can easily pass through the testicular blood-barrier. High melatonin in the testes increases spermatogenesis and strengthens sperm DNA (3). Lycopene is another antioxidant, shown to increase sperm count, viability and decrease oxidative stress particularly in men with varicocele. Lycopene can be found as a supplement, dosage recommendation is about 30mg daily. Lycopene can also be naturally found in tomatoes (4). Is having three testicles bad Most likely, yes. There is a very rare condition called triorchidism, with only about 100 reported cases (5). More likely, if you observe three masses in your scrotum, there is a varicocele, hernia or tumor. A varicocele is dilation of the veins that feed the testicles, the mechanism of testicular varicoceles is yet undefined but may be due to increased pressure in the abdomen or outflow obstruction. Outflow obstruction can occur due to the unique testicular vein anatomy. The left testicular vein is longer, the majority of which does not contain valves to prevent backflow, making left testicular varicoceles more common than right. Varicoceles can be dangerous for the testicle and the increased blood sitting next to the organ decreases both testosterone secretion and spermatogenesis via increased reactive oxygen species and heat (6). Inguinal hernias are an outpouching of the intestine through the muscles of the pelvic floor. These hernias occur from a weakened muscle layer combined with increased intraabdominal pressure, most commonly due to obesity. Obesity increases the risk of these hernias due to abnormal stretching of the muscle wall and increased intraabdominal pressure from fat pushing the bowel down, into the pelvic floor (7). This type of hernia, similar to varicocele, can decrease fertility. Finally, a third hard mass in the scrotum should prompt attention to obtain a physician evaluation for possible testicular cancer. Testicular cancer most often presents in young men, aged 15-35, as a painless slowly growing testicular mass (8). Is it healthy to massage your testicles Yes! Testicular massage not only feels good but is good for you! This practice can increase your sperm count by stimulating release of sperm into the epididymis to await ejaculation (9). Testicular massage also increases systemic testosterone by increasing release of testosterone during the massage (10). Furthermore, regular testicular massage can help you self-screen for testicular cancer, especially if you are under the age of 40, these can be silent cancers, which can be caught earlier in a more easily treated state if regular testicle inspection is performed. How to do a testicle massage Before beginning, you should know that the testicles can be easily injured with too much pressure or impact. When attempting a testicle massage, or "balls massage" as some call it, you should start off more gently than you think you should be, just to be safe. As you become more comfortable with massaging your testicles you can add additional techniques and pressure as needed. How to massage your testicles: holding the penis out of the way, gently roll each testicle between your fingers and thumb. You can both perform a milking action, aiming upward and a pill-rolling action.
